Complete the following form for membership to SACHSA and access to the membership areas of this website. Please read the SACHSA Constitution, Values, Statement of Faith, Policy Statements and Privacy Policy documents listed in the Membership menu to the right. Those documents will open in a separate browser tab or window.

If you prefer to mail in your membership form and payment, please click here to email the SACHSA Office and one will be emailed to you.

Once registration is completed you will received an email confirmation with instructions on how to complete the registration process and how to pay via Paypal

Dues before SEPTEMBER 1, 2010 (or if you are a new SACHSA member)-- $30.00

Dues after SEPTEMBER 1, 2010-- $30.00 + $15.00 late fee = $45

If you have never been a SACHSA member and you become a member after December 31, 2010, you pay $15.00.
